I'm in a bit of a different situation since my day job is writing. Although, its generally not my own reflection or thoughts. That's just the reality of writing for organizations.

Of course, everyone I know is aware I write for a living. Even still, they don't really understand what I do at work and don't call me a writer. A lot of my family and friends still call me a marketer, since that's where my background is but it's not what I do at all anymore.

However, the only person who knows I write on Medium, or run a pub for that matter, is my wife. I've kept it quiet from friends and family. I share my work on social media, as well as stories from my pub, but use accounts separated from my normal personal ones.
